This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Jerica McQueen, LMHC (“this practice,” “I,” “me”) is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice describing my legal duties and privacy practices, and follow the terms of the notice currently in effect.
You have the right to: request a copy of your records; request corrections to your records; request restrictions on certain uses or disclosures; request confidential communications; receive a list of certain disclosures; receive a paper copy of this notice; and file a complaint if you believe your privacy rights have been violated, without retaliation.
Process notes I keep separately from your clinical record receive additional legal protection and generally require your specific written authorization to release, except in limited circumstances required by law.
I reserve the right to change this notice and to make the revised notice effective for information already held as well as information received in the future.
If you believe your privacy rights have been violated, you may file a complaint with me directly or with the U.S. Department of Health and Human Services, Office for Civil Rights.

This page is the general public-facing rights notice only. Your individualized dollar estimate is generated separately, per client, after scheduling, not published here.
This is different from the Notice of Privacy Practices above. That notice covers your clinical and health information as a client. This one covers the ordinary technical side of visiting this website — things like cookies and third-party tools — whether or not you ever become a client.
This site does not use Google Analytics, Meta Pixel, Google Ads tracking, or any other marketing or advertising tracking tool. No visitor behavior on this site is tracked, sold, or shared for advertising purposes. If that ever changes, this page will be updated first.
The form on the Contact page sends your message directly to my own business email account — it isn't routed through a third-party marketing or form-relay company. After you submit it, you'll also get an automatic reply from me acknowledging your message and sharing my booking link. The form only asks for your name, email, and phone, and asks you not to include any health or personal details — we'll cover all of that once we connect directly.
Outside of the tools above, this website doesn't set its own cookies, doesn't ask you to log in, and doesn't store any information about you on its own. The public pages of this site only ever ask for your name, email, and phone — never clinical or health information.
Most browsers let you block or delete cookies, including third-party cookies, in their privacy settings. Blocking cookies may affect whether the scheduling widget works properly. You're always welcome to reach out and book by email or phone instead.
If the tools this site uses ever change — for example, if analytics is ever added — this page will be updated to reflect that before the change goes live.
